The Field Trip Permission Form must be completed and turned in (along with a copy of your health insurance card) prior to the first away game trip of the season.

In order for a student to leave an away game or competition with a parent/guardian (and not be required to return home on the bus with the rest of the band), a parent/guardian must complete the online form the day prior to the scheduled event. NOTE: Following an event, parents/guardians must report to the child’s assigned bus to “sign out” the student with a bus chaperone prior to them being released to leave. Students may not be checked out by a sibling or friend.
